OF THE TWO MAIN CATEGORISE OF HYPERTENSION -SYSTEMIC HYPERTENSION AND ‘PULMONARY HYPERTENSION – WHICH IS MORE DANGEROUS?

Simply put arterial [systemic hypertension is more silent and is the most dangerous hypertension than pulmonary hypertension these are the reasons pulmonary diseases are prominent complaints , the one who is afflicted with a lung [pulmonary]disease will certainly complain of a form of chest pain a kind of breathlessness, a form of irritating and disturbing cough, previous catarrh etc, many heart problem are silent even until the fatal damage is about to occur unless a competent hand to help in its detection.

And in respect of high blood pressure disorder it is unfortunate that the heart and the lung do work together as one unit,it is therefore sometimes difficult for many people to differentiate between a lung disease and a heart disease, strangely some doctor so called are bamboos.

This is partly because most illness of the heart affect the lung in disastrous way to the extent that even the incompetent doctor may become confused , so when their is, for example an acute adult respiratory distress syndrome [ARDS] occasioned by a a near fatal myocardial infarction [mi or heart attack] whose catalyst is either high blood pressure or an arteriosclerotic plaque of hypercholesterolaemia origin a hasty diagnosis of a lung disease like acute bronchitis or acute bronchial asthma may be entertained by the inexperienced house doctor this is because the symptoms of these disease are basically those of breathlessness and their signs are those of respiratory disease stigmata such as chest crepituus, central cyanosis and tacypnoea.

To the young physician or the inefficient doctor who may find it a big task to differentiate between cardiac asthma and bronchial asthma these are but symptoms and signs of respiratory [lungs]disease but not the illness which has no complaint, must have a way to express itself, heart disease tend to express themselves by simulating lung disease, heart diseases like high blood presure which is very silent until it is about to cause a near fatal disaster has but one way to complain through the lungs!hypertension is always silent until it has caused -it is about to cause heart attack or acute respiratory distress syndrome –otherwise know as cardiac asthma or acute pulmonary oedema.

On the other hand all lung disease have immediately and easily recognise symptoms and signs, pneumonia, for example disease will always produce some chest pain and cough if it must be called pneumonia, tuberculosis of the lung has its own characteristic ways expressing itself both to the layman and to the physician, emphysema thoracic, pleurisy flail, chest hyothrax, pnenmothrax haemothprax, pneumoconiosis bronchitis, bronchiectasis bronchial asthma, brochial carainnoma and other notable lung disorder have their ways of expressing themselves.

In every respect therefore the silent symptomless hypertension which is systemic arterial hypertension is the more dangerous disease than the more outspoken lung disease called pulmonary hypertension. Click Here to learn more

HOW CAN I DIFFERENTIATE ARTERIAL FROM PULMONARY HYPERTENSION

The differentiation between these two clinical conditions is the doctors problem, however it is important to know that pulmonary hypertension brings on difficulty with breathing and it is a lung disorder but arterial hypertension is a heart and blood vascular disease and it is symptomless.

YOU MEAN HYPERTENSION IS SYMPTOMLESS?

Yes hypertension does not even cause headaches. When headaches occur in the undiagnosed hypertensive , two things may be happening. One problem is the onset of a complication of the high blood pressure itself and the likelihood of the onset of an entirely new disease .

But do not confuse the majority of people with very high blood pressure of about 170mmhg systolic or more and 110mmHg diastolic or more are bound to suffer from occipital headaches at interval and may experience disturbed sleep patterns too [Occipital headaches are often experienced as heaviness and dull early morning aches about the junction between the back of the head and the neck ]

The headache is no symptoms at all but it is the evidence of an impending complication of high blood pressure. This means that headaches in high blood is symptoms of an impending brain damage,e.g stroke and when the hypertensive complain of central chest pain , the hypertensive is only about to have a heart attack. When the same hypertensive has repeated palpitation, his problem is not just illustration of the high blood pressure but an invitation to heart failure.

Hypertension has no symptoms , otherwise many who have had the ugly aspects of high blood pressure disorder would have had specific complaints ever before it causes disasters like stroke, heart attack, kidney failure, heart failure, sudden death etc. Many who have died from any of the foregoing would have long complained about their impending doom and these unfotunate folk might have been saved if hypertension had enough notable symptoms!

WHAT ARE THE BASIC ORGAN AND STRUCTURE AT RISK IN HYPERTENSION?

The organ at risk in any hypertensive heart disease condition include the brain, the ageing eye, the heart muscle pump itself, the thoracic aorta, the abdominal aorta, the pregnant womb , the kidney and their individual blood vessel. The aorta is a blood vessel that is anatomically divided into segment and it is considered to be the largest and the thickest blood vessel in the human body.

How are these organs damaged by a high blood pressure condition?

The blood supply to these organs are disrupted over time sometimes suddenly the damage to the organ occurs at their basic subunit level. This may occur in the brain cells [neurons],the heart muscle [myocardium] cells or the functional units, the nephrons of the two kidneys. It may affect the photo plate [retina] of the eye of the ageing adult or the pregnant womb of a susceptible group of women. The sudden or the gradual reduction in blood flow to these tissues and organs can cause a significant damage to the functional capabilities of these organs. The disruptions can also affect their structures in a very unpredictable way.

Whenever key organs like the heart, the kidney, the brain or the pregnant womb are damaged by disease the consequences are numerous and devastating. For example in several hypertension peculiar to some pregnancies – the condition which is called ‘preeclampsia or pregnancy –induced hypertension PIH – there is often a sudden disruption in the blood supply to the growing baby [foetus] and this may cause the demise of the lady and even jeopardize the existence of the pregnant lady . This loss of life through high blood pressure is a subject of concern in obstetics perinatology and in gyneacological practice and it may remain so for sometime to come .

Also coma [deep unconsciousness] resulting from a disruption in the blood flow to the brain caused by an elevated blood pressure problem is a severe blow to the brain. Some call it such fanciful names as cerebrovascular accident,CA (cardiac arrest) or stroke, but the truth is that many have died from this hypertension disaster . The problem is so common that many assume hypertension was born with some fellows in their families , that they must live it and accept it as part of their own fate and that they must die from it ! but no,this should not be.

However if high blood pressure does not succeed in damaging the kidney or in injuring the brain, it may successful put the heart itself out of action through heart attack or through hypertensive heart failure or through some other way.High blood pressure problem can be this devastating and it is almost always so in stroke as well as in heart failure.

Is the damage to these organs grave or mild?

Hypertension damages important body organs with impunity, the damage done may be reversible such as in preeclampsia, irreversible as it is the case in some stroke paralysis. This reversibility however depends on the degree of damage that has occurred at the time of the detection of hypertension.

For example the deeply unconscious [comatose] hypertensive who has just suffered a stroke has severe type of [brain] damage. He may wake up alive or open his eyes only at the other side of life. Some also suffer from a severe myocardial infarction [heart attack] and develop a sudden [acute] heart failure which means their problem was grave. But some who suffer stroke –paralysis only have weakness of one limb for a couple of days or weeks and they recover fully which means they had a mild variety of the consequence of their blood pressure.

There are people who suffer from repeatitive heart (angina pectoris) because of their elevated blood pressure but before they are down completely they recover after just a few days of bed rest. Theirs also is a mild variety on the whole, it means that the problem of high blood pressure is sometime mild and at other times it is grave.

Summarily then, it can be said that when the target organ is a delicate one like the kidney and hypertension is servere, the damage is often grave because the consequence may be an end stage of kidney failure which usually spells deaths for many. But when the damage is to a resilient organ such as the heart [muscle] itself and the blood pressure is not very high , the affliction is mild if it caused repeated heart pain [angina pectoris and nothing more.

WHAT DOES IT MEAN FOR SOMEONE TO HAVE A HIGH BLOOD PRESSURE [HBP]?

High blood pressure is a problem which the blood flow is impeded by some means other than which is detectable ordinarily. It is a clinical state of the heart and of the blood circulation in everyone, the heart acting as a mechanical pump and as an electrical pump combine in one , need to daily pump the blood around the body effectively, effectively and scrupulously . This fantastic made up combination of mechanic cum electrical pump system help the heart to function properly as a good pump which distributes vital material [wastes , oxygen, drugs, water, hormones and nutrients]to and from all the body organ as the case may demand it physiologically functionally.

But in the hypertensive the heart need to accommodate and fulfill more demand than it normally should, yet it must perform these daily tasks effectively and efficiently. This mean the heart of the hypertensive needs to put in more effort in order to achieve its daily tasks. The problem with an increased work demand for the heart is that the heart must increase its pumping capacity so as to be able to overcome the impedance to its successful blood distribution job.

Unfortunately the pressure to achieve the feat increase as the impedance and demand increase. Therefore a higher-than normal blood pressure occurs. And at any above normal high blood pressure is a risk to all important organ and structures of the body.

Therefore the person with a high blood pressure has a body whose demands far out weigh the abilities of his heart normal ability. So his heart must of necessity put up more effort in order to meet this demand,a task that must be done but which is very distressful to the labouring heart itself.

THE SYNONYMS AND TYPES OF HYPERTENSION.

Hypertension has several synonyms ‘high blood pressure Systemic hypertension arterial hypertension or hypertensive heart disease but the commonest variety of hypertension is know as essential hypertension’ strangely enough there is no part of the world that hypertension is completely absent but hypertension is a medical condition that tends to favour a people more than some others. this is the reason there are several names for the illness in different parts of the world .

For its popularity among the elite class and the socialites of our present world it is even on debate that is should be considered a symptoms –complex and not a disease state . The proponents say it is too common to be classed as a disease since it is even without any specific symptoms of its own yet it herald some detrimental disorder such as kidney failure or nerve paralysis , so , many regards it as a symptoms of a bigger disorder.

But I think illness incapacitate and annihilate. And hypertension can stylishly perform both of these tasks and it will do them very accurately too, to me illness are treated with drugs and high blood pressure is known to respond to several drugs.
Arterial hypertension or systemic hypertension is the common type. it affects the heart and the general blood circulation.

The unpopular hypertension is in various categories. This type affects organs and tissues of the body they have no direct link with high performance of the heart itself .a good examples is the type which affect the lungs and the blood flows to the airway . This is called ‘pulmonary hypertension .[pulmo’’ is latin word for the lungs]

There are some other unpopular types of hypertension too and they include ocular hypertension which means that the eyeball structure are under an increased influence of tension from the fluids bathing them .

This results is some degree of visual disturbance or even ,blindness a good evidence of ocular hypertension is the illness of the eye called glaucoma. Another beautiful subgroups of the unpopular variety of hypertension is the one which afflicts the liver. The liver may be diseased and it may be working under an increased pressure .

This means that blood flow is greatly impeded and its general workability is impaired . this type of hypertension is called portal hypertension. It is a notable killer among alcoholics hepatitis.

Also there are subdivision of the popular arterial hypertension and these include renovascular hypertension which is a representative disorder of the blood flow to the kidney and this condition gives rise to an elevated blood pressure . A good example of renovascula hypertension is the high blood pressure disorder that is associated with a narrowed blood vessel supplying the kidney [renal artery steno sis ] similarly renal hypertension is a problem primarily of the kidney which gives rise to an increased blood pressure level . It is represented by such kidney disease as p phritis olycystic kidney disease hypdronephrosis chronic glomerulone and pyelonephritis which are all linked to an elevated blood pressure as they progress .

There is post-traumatic hypertension too and it is high blood pressure which is sequel to an accident –often , a near fatal one . Also there is high blood pressure arising from defective ductless gland function. It is referred to as hypertension of endocrine origin . some other variant do exist of the popular class of arterial hypertension and they include the unstable blood pressure disorder such as labile hypertension and paroxysmal hypertension , but there are other two not well know subgroups of the popular arterial hiht blood pressure disorder that must be mentioned .

They are the arterial hypertensive heart disorder caused solely by pregnancy or that which has been corrected by surgery, these high blood pressure states are respectively called pregnancy –induced hypertension [PH] and surgically –correctable hypertension. All these are well described in the hand book –hypertension simplified by this same author,Dr Mercola Braden
